HomMed Health Monitoring System

Because patient care comes first, we provide in-home monitoring using the HomMed Health Management system. The technology has been made as simple as possible to use. Daily monitoring allows clinicians to detect actual or potential problems, eliminating unnecessary hospitalizations and ER visits.

Individuals struggling with chronic disease or other health problems, and their family members, have complex needs. Meeting those needs is one of the biggest challenges facing the health care industry today.

The HomMed Monitoring System has two components: the monitoring unit that is placed in the patient's home or clinical environment, and the HomMed Central Station. The Home Unit is a vital-sign monitoring and communication device that measures weight, blood pressure, heart rate, Sp02 and temperature. A Glucometer, ECG and other peripherals can be added for customized patient monitoring. The monitoring system and Central Station are FDA Class II Medical Devices.

Each day, a gentle, friendly voice guides you through the simple, three-minute procedure to collect all your vital signs. After the last of the vitals is taken, you are is asked up to 10 subjective questions to answer yes or no. These questions can be customized to assist in further evaluating your condition and quality-of-life measurement.

Your vital signs are automatically sent to the HomMed Central Station via an internal modem over your telephone line or a digital two-way paging system to the central station. No patient demographics are attached to the data. The HomMed Central Station's proprietary software tracks and trends the data collected and displays it in a manner that allows for easy analysis.

The HomMed Central Station is designed to give healthcare providers the opportunity to focus on the patients, not the operation of a complicated system. The HomMed software is an information collection and management system that allows clinicians and physicians to retrospectively analyze patient vital signs and other test results. Applications have the capabilities to track and trend patient data, record and view nursing / physician notes, log patient contacts, and generate printed or faxed reports.
